Application Packets

As we look to the VIM Teams going to Haiti this Fall and next Winter, the Executive Committee (consisting of team leaders and officers) has developed the following goals as we build the teams:

  • Recruit the best team possible that will work well together and provide the various skills necessary
  • Encourage new people to go on a VIM team
  • Encourage young people to go on a VIM team
  • Provide opportunities for people from within the boundaries of the new Susquehanna Conference and Upper New York Conference but outside of the historic boundaries of the Wyoming Conference to join a team
  • Involve as many people in the Haiti Partnership as possible without excluding anyone unnecessarily
 

To achieve these goals, we have also developed this process for receiving applications and making decisions about teams:

  • The first step is to fill out and return an application to the Registrar with a $150 deposit to be considered for membership on a team.
  • The second step is that our goal is to finalize teams 120 days before each trip. Therefore applications should be sent in as early as possible. Members will be chosen to best achieve our goals listed above. When a person is chosen for a team, they will be contacted by a team leader before buying their plane ticket.

APPLICATION PACKETS TO DOWNLOAD

The application packet is in Adobe PDF format and can be downloaded by clicking on the link below.  The packet contains all necessary forms.
 
The application form and GBGM Insurance form may be filled out on your computer using Adobe Reader.  PLEASE NOTE that once you begin to fill out the forms on your computer, you cannot save the forms.  They must be completed and printed out in one session. Or, you may print the forms out and fill them out by hand.
 
All forms must be printed out and signed regardless of whether you have filled them out by computer or by hand.
 
If you want to apply for two different trips, you must submit a separate packet for each trip.
 
Note: the Pastor's Recommendation form in each packet is for first-time volunteers only.
 
You must have Adobe Reader installed on your computer to read these forms.

PLEASE NOTE: We have changed our application packets.  At the top of each application is a list of all the upcoming trips.  You are asked to specify your first and second choice of trip dates. 
 
Regardless of what you may read elsewhere, all checks for trip applications should be made payable to "Upper New York Conference" (and please spell out "New York").
